In Deus Ex: Invisible War, the difficulty level (or simply difficulty) is a game setting that is selected at the start of a new game. Difficulty level determines how deadly in-game combat will be. The player may select from four difficulty levels: Easy, Medium, Hard, and Realistic.

Difficulty level affects multipliers associated with four aspects of gameplay: damage taken by the player character, damage taken by enemies, enemy accuracy, and ammo per clip.[1]

As shown above, realistic is more challenging than hard only in terms of damage taken by the player and the amount of ammo per clip. Realistic is less challenging than all other difficulty levels in terms of damage taken by enemies, and equal to normal in terms of enemy accuracy.
